The Journal is a notebook the Protagonist carries with him. It can be accessed via the J key.
The Journal currently consists of four accessible tabs along the sides: ME, JOURNAL, ITEMS and KEYS. Five other tabs are theoretically in the game (Notes, Locations, People, Creatures and Quest Items), but they cannot be accessed, and are most likely being in development.
The ME tab shows what skills the Protagonist has gained, both positive and negative, but there is no description for each skill, probably due to an oversight. Thus this tab is rather useless.
The JOURNAL tab shows you important parts of conversations and events.

The ITEMS tab shows what quest items and story items you've picked up through out the game. You can also view them by clicking on the names.
The KEYS tab shows what keys you've collected.
